令和7年度入学予定者を対象にした就学時健康診断について

★就学時健康診断 

令和7年度入学予定者を対象にした就学時健康診断を行います。 

対象者には通知文が届いておりますので、ご確認の上ご来校ください。 

やむを得ない理由で指定の受付日時を変更したい方は、就学時健康診断通知書に記載されている二次元コードより、電子申請にて変更手続きをしてください。 

検診当日、体調不良等で欠席される場合は、学校へご連絡をお願いいたします。 

日付：令和6年11月14日（木）　 

受付時間：通知文記載の時間にお越しください。 

　　　　（13：25～、13：40～、13：55～、14：10～） 

持ち物：通知文、就学時健康診断表（記入漏れがないようにお願いいたします）、上履き、下足入れ（お子様、保護者様ともに必要です）、筆記用具 

※変更の場合はHPでお知らせいたしますので、ご確認をお願いいたします。 

※自転車でのご来校はご遠慮ください。近隣へのご迷惑となりますので、学校周辺への駐輪もご遠慮いただきますようお願いいたします。

登校したら、玄関で健康カードをチェックしています。

Students’ Health Cards are checked at the entrance upon their arrival.

 
 カードを渡すときの「おはようございます」のあいさつも、大きくなってきました。
 
 The students’ greeting voice for “Good Morning” has become larger when submitting their cards. 
 

また、「お願いします」と言って渡してくれる子もいます。

Some of the students even say “Thank you for checking” when submitting it.

 
 

都内での感染拡大を受け、カードの中の「家族の健康」欄への記入も忘れずにお願いします。

 

 
 

Due to the increased spread of infections in Tokyo, please fill out the box for the family members’ health on the card.

 

熱中症予防のため、登校中はマスクを外している子は、玄関に入るときにマスクを着用させています。

忘れずに持ってきましょう。

In the case that the students come to school without wearing masks for a purpose of heat stroke prevention, they will be asked to wear it as they enter the school.

Please make sure to have your child bring a mask with them.
